I, THE UNDERSIGNED, accept the following terms and conditions of membership to the Joondalup United Football Club:
- VIOLENT AND OR ABUSIVE BEHAVIOUR OF ANY KIND WILL NOT BE TOLERATED BY JOONDALUP UNITED FOOTBALL CLUB.
- All players, parents & coaches will be bound & must abide by the Joondalup United FC Charter and Joondalup United FC Constitution,
failure to do so may result in suspension or expulsion from the club. - All players, parents & coaches must abide by the By Laws & the Rules of Competition of Football West.
- All new players must provide a copy of a birth certificate or previous players supply FW registration number.
- Players will be placed into teams in accordance with player numbers and allocation of divisions by Football West.
- All applications for Kidsport Funding assistance must be advised to the club along with approved voucher code.
- That ALL FEES must be paid in FULL and players must complete an online Football West Registration Form to be eligible to play.
- NO REFUNDS will be given unless exceptional circumstances arise & will be at the committee’s discretion.
- Limited insurance cover only is available through registration with Football West. Players play at their own risk.
- I give permission for myself/my child, named on the form, for photographs to be published on the Joondalup United Football Club’s website, Facebook pages and associated social media accounts.
